If your appointment is due to a problem of the knee, shoulder or elbow, you will also need to complete a questionnaire about how the problem began, any pain you are having, and activities that are now difficult to perform. Please select the appropriate form for the list below, print and complete it and bring it to your appointment with Dr. Cole.

It is important that all questions are answered completely. The information you provide also helps us to track your progress after treatment.
